OK, so after some time I was able to figure it out a bit and play with it.So here in text field write text that is same in every "incoming call" window. For example text "Mute" or "Keypad" should be same everytime.
Into apps filed choose truecaller and confirm that counfiguration.
After this done. Add some simple action like notification, save macro and try if you get that notifucation when someone's calling.
OK, so after some time I was able to figure it out a bit and play with it.
I still need some help.

Now from here how do I extract only the name (Y) on "element texts"?
This was helpful, but still has issues:
1) It only speaks the first name (first half of the name) and not the last name. (I guess something with regex)
2) It only works when the screen is unlocked, is there a solution to this
