Aha - yes, adding that permission makes it work again.
It's not obvious as it doesn't appear on the page that is shown when you tap "Check wifi permission" on the Xiaomi helper app, you have to go into the "App info" pages to find it.

I updated Macrodroid today and that asked me to update to MD Helper V1.10 & MD Xiaomi helper 1.2 as well.
Now I have the same problem as you, Bluetooth can't be switched off, it worked with the last version.

I have an Xiaomi Redmi note 10, a couple of days ago it updated to MIUI 13 / Android 12.
Now I've noticed that it disables WiFi overnight if it's not in use.
It seems that between 2300hrs & 0600hrs, when I lock the screen it disables WiFi about 20 minutes later.
A real pain as I have a couple...
Just thought I'd report back on this, the macro works fine on my phone now, I guess MIUI didn't like it waking up every second to check if the device was unlocked, seems to tolerate the 10 second loop to flash the torch fine.
BUT - it still gets stopped on my partners phone (same model and OS...
I think you need another 'Torch off', just after the 'Cancel macro actions'.
Otherwise you can still leave the torch on if you get the timing wrong.
(Does that work, or does it stop immediately and not carry on?)
Yeah, that's pretty much what I've got (it also checks for battery > 20%)
But you still need the separate macro that catches 'screen unlocked' and stops the loop.
Otherwise it's possible to do [ unlock, view the notification, lock again ] within the 9 seconds wait so the loop doesn't stop.
