I would like to very strongly second this request for a trigger (or an option within the existing trigger) that would fire when any app opens or closes.

I would however suggest a 'select all' method would not be much help as the user would have to keep updating the trigger settings in order to catch new apps.

If you don't mind using a plugin (and giving it permission to pretty much see everything you do) you could try TouchTask plugin's "Screen update" trigger, i believe this can write the foreground app name/package to a variable and you can then have it do whatever you want with that information.
